Patient Summary

Mr. Jonathan Olsen presented for a virtual consultation seeking both functional and aesthetic improvements to his nose. Following orthognathic (jaw) surgery 10 years ago to correct an underbite, he has noted ongoing functional breathing difficulties, specifically a collapse of the nasal sidewalls upon deep inhalation, leading to chronic mouth breathing, mouth taping, and snoring.

Aesthetically, he wishes to refine the protruding nasal tip to achieve a more balanced facial profile. His medical history notes childhood epistaxis (nosebleeds) treated with cauterization—which is currently stable—and nasal rosacea, which is well-managed. During the consultation, Dr. Palapong also observed hyponasal speech, further indicating restricted airflow.

Grafting Expertise
Specialized Dual Expertise: As an ENT-trained facial plastic surgeon, Dr. Palapong intimately understands both complex internal airway dynamics and external aesthetics.
Advanced Structural Reconstruction: Highly experienced in utilizing autologous rib cartilage for robust, long-lasting nasal framework support.
Bespoke Diagnostics: Emphasizes individualized diagnostic imaging (PNS CT Scans) to tailor the surgical approach to your unique anatomy prior to making an incision.
2
Doctor's Assessment & Recommendation
Clinical findings from the initial consultation
Internal Nasal Valve Collapse: Visual and auditory cues (hyponasal voice) strongly suggest significant structural weakness causing the sidewalls to collapse during inhalation.
Diagnostic Requirement: A Paranasal Sinus (PNS) CT scan is required to fully map the internal airway and sinus health prior to finalizing surgery.
Reconstructive Approach: An Open Rhinoplasty is recommended to provide full exposure of the nasal framework for accurate, targeted modification.
Rib Cartilage Grafting: Given the structural collapse, a rib graft is essential for building a strong, stable "roof" and sidewalls to permanently prevent airway blockages and refine the tip.

Your healing journey
Days 1-3
Maximum swelling and mild discomfort. Mouth breathing will be required while internal packing or splints are in place. Rib donor site may feel sore.

Removal of external nasal splint and non-dissolvable sutures. Check-up of the rib donor site incision.
Weeks 2-4
Significant reduction in visible swelling. Gradual return to normal daily activities and light exercise.
Months 6-12

Recommended Length of Stay in Bangkok

We strongly recommend planning to stay in Bangkok for a minimum of 10 to 14 days post-surgery. This allows ample time for splint and suture removal, ensuring the initial healing phase of both the nose and the rib donor site is complication-free before you fly.
